This can be used when wide awake doing physical therapy activities when laying down or even in other positions since this is adjustable to the way your body should be positioned. Just took a picture on the lower bottom of this item showing 2 tiny things when I went underneath it to check it out. I don't know what those 2 tiny things are sticking out. But showed the picture to the staff there the other day. They intend to send someone to check this out.
